About The Position

This position will provide intensive case management and advocacy services to individuals aged sixteen and over who are experiencing mental health, dual diagnosis and/or addictions and who are vulnerably housed or homeless. Through a collaborative approach, this position will facilitate recovery and assist service users who are experiencing significant difficulties in daily functioning to improve their quality of life. Tasks will include direct service interventions, clinical support and community development. Daily travel within York region will be required.

Requirements

Bachelor’s degree in social work or related discipline (e.g., Health Sciences).
Minimum 3 years of related experience in social services with adults who have a serious mental illness, dual diagnosis, developmental disability, or addictions.
Broad experience with individual, family, and systems advocacy.
Supportive counseling experience.
Crisis prevention and intervention experience.
Experience in providing support around housing and/or homelessness.
Sound understanding of the population and relevant legislation.
A valid G Driver’s License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.
Ability to provide a police clearance that meets agency standards.
Must reside within a reasonable daily commuting distance of the office and/or assigned service area and be able to attend the office as required for operational needs.
Ability to provide proof of a primary series of a COVID-19 vaccine authorized by the World Health Organization, or provide a written explanation of exemption.
