About The Position

Essential Duties and Responsibilities:

Provide face-to-face substance abuse psychosocial assessments
Daily review and update of completing assessments, and other duties assigned
by the program director
Facilitate patient referrals for additional services consistent with assessed
needs and physician recommendations.
Meet patients for follow-up reviews or updates as needed or required
Provide brief interventions, stabilization services and supportive counseling
to patients
Under the direction of the program director, the care coordinator will
provide patients with medication education print-outs that are focused on
understanding illness, symptom management and medication compliance
Complete and file documentation as required in a timely manner

Other Responsibilities:

1. Adheres to downtime procedures.
2. Adheres to established departmental policies, procedures, and objectives.
3. Performs both essential and non-essential job functions in a safe manner as
identified by Humboldt Park Health.
4. Demonstrates an understanding of and models the mission and core values of
Humboldt Park Health.
5. Enhances professional growth and development by accessing educational
programs, job related literature, in-service meetings, and
workshops/seminars.
6. Checks hospital email daily.
7. Performs other related duties as assigned.
