Mental Health Care Coordinator / Triage

About The Position

Responsible for providing a broad range of coordinated services to mental health/AODA patients and families in cooperation with physicians, nursing, psychotherapists, social workers and other professional interdisciplinary staff and community agency personnel, both inpatient and outpatient. Uses a collaborative process to assess, plan, implement, coordinate, monitor and evaluate options and services to meet individual patient health needs. Focus on triage/assessment, planning/referral, facilitation, patient advocacy, and patient/family education, crisis management and intervention and assisting psychiatrists with patient treatment needs. Ensures that initial patient intakes and referrals, triage intakes to most appropriate level of care and complete intakes are completed according to department policy. Demonstrates management of crisis situations in an empathetic and knowledgeable manner. Assists psychiatrist/APNP with phone inquiries regarding psychotropic medications. Assists staff psychiatrists with required activity (e.g., medication groups, setting up prescriptions, preliminary work in inpatient consultations, completing paperwork...). Arranges for inpatient psychiatric/AODA stays for identified patients. Acting as agent to MD/APNP for calling in scripts to pharmacies. Documents all activities according to JCAHO and State of Wisconsin regulations and standards. Completes and presents proper billing forms to staff secretary. Participates in planning, development and presentation of community and/or health system programs or treatment services. Demonstrates emphasis satisfaction. on internal/external customer Attends and participates in staff meetings, case reviews and strategic planning sessions. Other duties as assigned by the Department Director. Develops and implements an efficient and effective patient plan of care through collaboration with the interdisciplinary team. Be available to assist with coordinating care conferences needed. as Active participant in department utilization review committees. Assists in staff coverage as needed during absences, working collaboratively with the patient, family and interdisciplinary team across the continuum of care. Attends and participates in all staff meetings, case review or strategic planning sessions. Assists with on-call program.

Requirements

  • Master's Degree in Mental Health, Counseling, Human Services, Social Work or related field.
  • Three thousand hours postgraduate experience preferred.
  • An understanding of DSM-V diagnosis and treatment options.
  • Experience assessing, triaging, referring and following patient care initiatives.
  • Ability to function within both roles of lead and team member of multidisciplinary team.
  • Excellent written and verbal communication skills.
  • Excellent interpersonal skills.
  • Ability to exercise sound judgment.
  • Excellent organizational and problem-solving skills.
  • Adheres to established health system and Counseling Care Center department policies, rules and regulations.
  • Maintains ethical conduct and keeps confidential personal and medical information about patients.
