About The Position

WestCoast Children’s Clinic is a non-profit community psychology clinic in Oakland, California, providing mental health services to children, youth, and families in the Bay Area. The organization is client-centered, trauma-informed, collaborative, and committed to justice and equity. The Mental Health Care Coordinator, also known as the Intensive Care Coordinator (ICC), is responsible for assessing, developing, coordinating, implementing, and overseeing intensive behavioral health, community-based care plans for clients with complex behavioral health, medical, developmental, and psychosocial needs. The ICC facilitates the formation of a Child & Family Team (CFT) by bringing together family, natural supports, and formal system partners to address goals and ensure lasting change. This role involves clinical judgment in evaluating client needs and strengths, determining interventions, coordinating services, managing crises, linking to resources, building skills, and developing individualized care strategies. The ICC coordinates and facilitates CFT meetings and monitors progress toward youth goals. Services are provided in virtual and in-person settings, including client homes, schools, and community locations. This position primarily serves youth and young adults in the C-Change program, which focuses on supporting at-risk youth, including those identified as sexually exploited minors, by promoting stabilization, reducing mental health symptoms, enhancing self-efficacy and self-determination, building community, and connecting youth with resources. The C-Change program offers mental health screening, advocacy, clinical case management, and psychotherapy to youth up to 21 years old throughout the Bay Area.

Requirements

  • At least 2 years of professional experience working directly with children and/or families from a mental health framework in behavioral health, care coordination, intensive case management, social services, healthcare coordination, or related clinical services.
  • Group, training or treatment team facilitation experience and skills are required.
  • Bachelor's or Master's degree in Psychology, Social Work, Human Services, Counseling, or a related field required. Candidates with significant relevant professional experience in mental health or behavioral health services may be considered in lieu of a degree.
  • Demonstrated experience working with adolescents/minors with dual diagnosis, high mental health needs, and/or have experienced child sexual exploitation (CSE).
  • Ability to travel by car to meet with clients, generally within a 90-mile radius of the WCC office. Clean CA DMV record, current car insurance and a vehicle; travel is required.
  • Ability to apply professional judgment in ambiguous and evolving situations.
  • Strong crisis assessment and intervention skills.
  • Passionate about working with foster youth, families & young adults who have complex traumatic histories.

Responsibilities

  • Conduct comprehensive biopsychosocial, behavioral health, risk, and functional assessments for clients with complex and high-acuity needs.
  • Support the development, implementation, and monitoring of individualized care plans using evidence-based and trauma-informed practices.
  • Evaluate clinical progress and modify interventions based on ongoing assessment and professional judgment.
  • Regularly schedule, organize, and facilitate CFT Meetings with youth/families and their community stakeholders.
  • Work collaboratively as part of a multidisciplinary treatment team to support youth and families with developing and making progress toward their goals.
  • Manage a caseload of up to 19 clients, ensuring 50% (20 hrs/week) is dedicated to providing behavioral health services to youth and families.
  • Uphold ethical and legal responsibilities, complete high-quality documentation, link youth to resources, support skill building, assess needs, strengths, and provide crisis intervention and de-escalation.
  • Represents our mission, philosophy, and upholds ethical and legal responsibilities as a mandated reporter.
  • Independently and collaboratively assess and respond to behavioral health crises, safety concerns, and urgent client situations.
  • Exercise professional judgment in determining appropriate interventions, escalation procedures, mandated reporting obligations, and emergency referrals.
  • Be well-organized, manage time/schedule effectively, prioritize and complete timely documentation.
  • Provide services in a variety of community settings such as homes, schools, parks, shelters, other agencies, etc.
  • Understand the value of building healthy relationships to help youth navigate complex systems and how trauma can impact youth and team dynamics.
  • Use supervision effectively.
  • Be self-reflective and accountable to growth areas.
  • Be passionate about learning and participating in ongoing training provided by WCC.
  • Participate in individual and group supervision, program meetings, and trainings consistently.
  • Complete daily progress notes and other clinical documentation that complies with policies and procedures for meeting EPSDT Medi-Cal and other billing requirements.
