This position works as part of an interdisciplinary team to provide triage and outreach services that are inclusionary and flexible for individuals with mental illness, substance use and/or chronic medical conditions. The role involves conducting mental and physical status assessments, and assessing the client’s physiological, psychological, sociocultural and spiritual needs to determine priority of care requirements based on client needs, availability of resources and best practices. The nurse will provide counseling and assist clients in reducing repeated contact with and use of acute and community healthcare services/programs/resources. This role also involves assessing high risk situations and providing crisis intervention. Collaboration with the interdisciplinary team is key to developing, implementing, and adjusting the client’s care plan to meet the client’s need for integrated services and continuity of care across the continuum. The position facilitates the movement of clients between acute care, community agency support sites, and designated health centers. Participation in case conferences/meetings with members of the interdisciplinary team and assisting clients with attendance to needed services such as medical appointments are also core functions. The practice aligns with the British Columbia College of Nurses and Midwives (BCCNM) standards (professional & practice) and code of ethics for registered nurses or registered psychiatric nurses, and operates within the scope of practice as set by the organization, per VCH policies and guidelines/clinical practice documents and within own competence. This practice is aligned with the vision, mission, goals and values of the organization as well as with the concept of the resident/client and family centred care model. The Inner City Intensive Case Management Teams (ICMT) provide a support role for people who experience severe substance abuse problems, chronic unmet medical needs, and multiple barriers for accessing services. Services are offered on a completely outreach basis with the goal to stabilize clients and connect them to appropriate long-term services. VCH has received approval from the BC Human Rights Tribunal to restrict hiring for this position to individuals who self-identify as women (including transgender women).
