The Case Manager for Adult Mental Health Services provides case management and clinical services for individuals with mental illness, substance use disorders, and/or developmental disabilities and their families. Services include supportive counseling, individual and family support, crisis intervention, assessment, Individualized Service Plan (ISP) development, psycho-education, and referrals to clients and their families. The role involves developing and maintaining working relationships with individuals served, families, additional service providers, and other community agencies to ensure quality services. The Case Manager works as part of a treatment team to ensure services and resources are coordinated among providers to help individuals served live and function optimally in the community.
