About The Position

The Memory Care Program Coordinator develops, coordinates, and supports person-centered programming for residents living with Alzheimer's disease and other forms of dementia. This position partners with interdisciplinary teams to promote meaningful engagement, enhance resident quality of life, educate staff, and uphold the philosophies and standards of the Memory Care Program.

Requirements

  • Experience working with residents living with Alzheimer's disease or other forms of dementia in a long-term care setting is required.
  • Strong knowledge of person-centered dementia care, dementia-related behaviors, interventions, communication techniques, and best practices for supporting residents living with Alzheimer's disease and other forms of dementia.
  • Ability to educate, observe, and support staff through both formal training and one-on-one coaching on dementia care principles and resident-centered approaches.
  • Excellent interpersonal communication skills with the ability to build positive relationships with residents, families, and interdisciplinary team members while demonstrating empathy, compassion, and professionalism.
  • Experience developing or supporting meaningful activities, social enrichment, and engagement opportunities that enhance residents' quality of life.
  • Ability to collaborate effectively with nursing, therapy, social services, activities, leadership, consultants, and regulatory agencies to promote exceptional resident care.
  • Strong teamwork skills with a commitment to delivering excellent resident care through collaboration, accountability, and person-centered practices.
  • Bachelor's degree in Social Work, Therapeutic Recreation, Psychology, Human Services, Healthcare Administration, or a related field is preferred.
  • Experience working directly with residents living with Alzheimer's disease or other forms of dementia in a long-term care, skilled nursing, assisted living, or memory care setting is required.

Nice To Haves

  • Previous experience in Memory Care, Social Services, or Activities & Recreation is strongly preferred.
  • Professional certification in dementia care or Alzheimer's care is preferred.
  • Passion for geriatric advocacy and a commitment to enhancing the lives of seniors through person-centered dementia care.
