About The Position

The Memory Care Coordinator is responsible for the overall coordination, implementation, and oversight of the memory care program within the assisted living community. This position ensures residents living with Alzheimer’s disease, dementia and other cognitive impairments receive compassionate, person-centered care that promotes dignity, engagement, safety and quality of life. The coordinator collaborates with the interdisciplinary teams, families, healthcare providers, and community staff to maintain regulatory compliance and deliver exceptional resident services.

Requirements

  • CPR certification required
  • Ability to meet state-specific assisted living and dementia training requirements.
  • Knowledge of Alzheimer’s disease, dementia, and behavior management techniques
  • Excellent interpersonal, organizational, and communication skills.

Nice To Haves

  • Associates degree, bachelor’s degree preferred in healthcare, social services, recreation therapy, nursing or related field preferred.
  • Maintain active clinical license in respective field if applicable
  • Dementia Care Certification is required within 3 months of hire.
  • Previous supervisory or leadership experience preferred.
  • Minimum 1-2 years of experience working with seniors, dementia care, or memory care programming preferred.

Responsibilities

  • Develop, implement, and oversee evidenced-based memory care programming and activities tailored to residents’ cognitive abilities and interests.
  • Monitor resident behaviors, functioning, and psychological well-being and communicate changes to the clinical team.
  • Assist with development and review of resident service plans and assessments
  • Promote resident dignity, independence, choice, and meaningful engagement.
  • Implement behavioral interventions, redirection techniques, and non-pharmacological approaches to dementia-related behaviors.
  • Participate in move-in assessments and transition planning for memory care residents.
  • Support fall prevention, wandering prevention, and resident safety initiatives.
  • Provide ongoing education and coaching to staff regarding dementia care best practices, communication techniques, de-escalation and behavior management.
  • Manage and oversee the experience team
  • Conduct competency observations and reinforce person-centered care approaches.
  • Assist with scheduling and coordination of memory care staff assignments as directed.
  • Promote a positive, team-oriented work environment
  • Participate in hiring and onboarding, and performance management processes as assigned.
  • Serve as a primary resource for families regarding memory care services and resident concerns.
  • Conduct family meetings and provide education on dementia progression, behaviors, and interventions.
  • Maintain professional and compassionate communication with residents’ responsible parties and healthcare providers.
  • Ensure compliance with state regulations, assisted living standards, and community related policies related to memory care services.
  • Maintain accurate documentation regarding resident participation, behaviors, incidents, and interventions.
  • Participate in quality assurance, audits and survey preparedness activities
  • Report concerns related to resident abuse, neglect, exploitation, or safety per community policy and regulatory requirements.
  • Maintain a safe, structured, and therapeutic environment for residents with cognitive impairment.
  • Monitor memory care neighborhood safety features, including secured access systems.
  • Ensure infection prevention and emergency preparedness procedures are followed.
  • Support dining services and daily routines that encourage resident success and comfort.
