About The Position

Southern Utah Veterans Home in Ivins, Utah is seeking an enthusiastic and dedicated Memory Care- NOC Shift -LPN or RN to join our outstanding team! The Licensed Practical Nurse (LPN) or Registered Nurse (RN) will provide compassionate and skilled nursing care to residents in the Memory Care unit during the night shift. This role requires a dedicated professional who can deliver high-quality care tailored to the unique needs of veterans with memory impairments, ensuring their safety, comfort, and dignity.

Requirements

Current and valid LPN or RN license in the state of Utah.
Strong clinical assessment and critical thinking skills.
Ability to work independently and as part of a multidisciplinary team.
Excellent communication and interpersonal skills.
CPR certification required or willingness to obtain upon hire.
Ability to work night shifts (NOC) as scheduled.

Nice To Haves

Experience in memory care, geriatrics, or long-term care preferred.

Responsibilities

Provide direct nursing care to residents in the Memory Care unit during the night shift.
Monitor and assess residents’ physical and mental health status regularly.
Administer medications and treatments as prescribed by physicians.
Document resident care accurately and timely in accordance with facility policies.
Collaborate with interdisciplinary team members to develop and implement individualized care plans.
Respond promptly to resident needs, emergencies, and behavioral changes.
Ensure a safe and therapeutic environment for residents with memory impairments.
Provide support and education to residents and their families as appropriate.
Adhere to all regulatory and facility standards related to nursing practice and memory care.
