The Memory Care Manager will report directly to the DON and will assist with coordination of resident care by providing guidance to caregiver staff in accordance with the organization's policies and applicable laws. This position is responsible for efforts tied to the direction, coordination, and evaluation of resident care.

Requirements

  • Must hold a current and unencumbered license as an RN (preferred) or LPN in South Dakota.
  • Capable of exercising good judgment to ensure a safe, comfortable, and secure living environment for the residents.
  • Able to lift, push, and pull 30 lbs. of weight without risk of injury.
  • Must submit to a TB test in accordance with state regulations.
  • Must be able to pass a background check.

Responsibilities

  • Ensure the well-being and protection of every resident through the delivery of high-quality care.
  • Promote and support a positive work environment focused on team building and collaboration.
  • Provide on-call backup to community when not on duty.
  • Maintain confidentiality of all personal or health information concerning residents and staff.
  • Ensure that all resident needs are met.
  • Support and supervise clinical staff.
  • Ensure that any incident reports are completed properly and promptly.
  • Prepare necessary medical paperwork for all scheduled resident appointments.
  • Check call systems to ensure proper functioning and timely alert response.
  • Assists with compliance in meeting regulations and be available for state licensure surveys.
  • Maintain resident files and appropriate documents.
  • Assist Medication Aides and CNAs in service delivery and training.
  • Notify care staff and DON/RN of any new provider orders and medication changes.
  • Perform inventory management for medication administration including new medications from the pharmacy. Complete medication setups.
  • Monitor ECP system to communicates any new/updated information to staff.
  • Perform glucose testing and insulin administration draws for applicable residents.
  • Completes and ECP changes per provider orders , and communicate changes to care staff.
  • Destroy medications per policy and regulatory requirements.
  • Chart appropriately on residents.
  • Assist with resident ADLs including bathing, dressing, and grooming.
  • Attend all staff meetings and 100% of all required training.
  • Complete post fall evaluation per policy.
  • Assist with serving meals.
  • Assist with picking up shifts on an evenly-distributed schedule among nursing staff.
  • Actively available for on-call rotation.
  • Schedule care conferences and update care plans in accordance with regulations.
  • Review, edit, and create nursing policy drafts to be reviewed and finalized by the Administrator/DON.
  • Develop and update resident emergency book for memory care.
  • Coordinate a dementia support group to provide ongoing education to internal and external resident family members.
  • Other duties as needed.
