About The Position

WelbeHealth’s PACE (All-Inclusive Care for the Elderly) program provides seniors with the opportunity to continue living in their homes and in their communities. Our innovative and comprehensive range of medical services to participants is what ignites our passion to treat the whole person and not the symptoms! We employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care. The Medication Support Assistant works as part of the clinical team, supporting the Clinic Nurse to ensure participants receive their prescriptions in a timely manner. This role focuses on ensuring key participant information is updated and accurate in all systems, as well as providing instructions to participants living in private homes or long-term care facilities regarding the packaging, delivery, and refill processes of all medications. This role is different because the Medication Support Assistant at WelbeHealth: Has the opportunity to make positive changes in the way we process medications and answer questions about medications as part of our new pilot program Plays a critical role in supporting a growing, mission-driven healthcare organization, ensuring vulnerable seniors feel supported and have access to high-quality care

Requirements

  • High school diploma or equivalency

Nice To Haves

  • Experience as a medication medical assistant, medication technician, or pharmacy technician preferred
  • Two (2) years of experience in a clinical setting with a frail or elderly population preferred
  • Experience working with frail or elderly populations

Responsibilities

  • Ensure prescriptions are filled in a timely manner to avoid gaps in prescription refills as new participants enroll with WelbeHealth
  • Ensure participant information, such as enrollment status and delivery address, are kept up to date in EireneRx and other systems as required
  • Address medication requests, issues, and questions, escalating as needed
  • Arrange medication delivery from local pharmacies or from the center
  • Manage medication orders and changes for participants in assisted living facilities and skilled nursing facilities as needed
  • Coordinate arrangements for diabetic supplies and equipment for new participants
