Medication History Specialist, Acute care
About The Position
This position is located in Fresno/Clovis CA. The Medication History Specialist works under the supervision of a physician or pharmacist to support medication reconciliation by improving the accuracy of the patient's medication history. This role leads to improved patient safety and satisfaction. The specialist obtains accurate medication history by collecting medication record information from multiple sources and entering current patient medications into the hospital's electronic medical record for utilization by the nurses and physicians responsible for admitting and discharging patients from the hospital.
Requirements
- High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required.
- RXTECH - State of California Pharmacy Technician License required.
Responsibilities
- Support medication reconciliation by improving the accuracy of the patient's medication history.
- Obtain accurate medication history by collecting medication record information from multiple sources.
- Enter current patient medications into the hospital's electronic medical record for utilization by nurses and physicians.
