The Medication History Specialist is responsible for completing area-specific work according to standard processes and procedures, adhering to policies. This role serves as an expert with equipment and technology in a specialized area, problem-solving technical issues as they arise. The specialist ensures documents and records are accurate and completed in a timely fashion, and supports methods of communication and coordination with patients and the care team. They identify quality improvement opportunities based on their specialized skill set to ensure safe, effective, and efficient services and patient care. The role provides input to and supports the implementation of operational changes, advancements, and site and system standardization. The Medication History Specialist participates in site or system specialized work teams, committees, and councils as applicable, and complies with legal, regulatory, and accreditation standards. They keep current with changes in the specialized field and collaborate with department leadership to adopt best practices, maintaining updated knowledge and skills while contributing to the education of others. The specialist contributes positively to the execution of pharmacy department initiatives and accepts assignments of tasks from other pharmacy teammate roles when workload dictates.
