As an integral member of the Emergency Department team, the Medication History Specialist (MHS) is responsible for working with patients identified for admission to the hospital to obtain and document the best possible medication history. This role is essential for ensuring high-quality and safe care by reducing medication duplications, omissions, and potential errors. The information gathered will be accurately entered into the computer system and used by physicians, pharmacists, nurses, and other healthcare providers as input for treatment plans and medication orders during hospitalization, as well as for discharge planning for post-hospital transition and care. The MHS must be capable of passing the medication reconciliation competency test for Pharmacy Technicians and annual competency and quality assurance metrics. This role requires strong interview and inquiry techniques to compile the best possible medication history from patients and families. The specialist is responsible for entering all home medications into the electronic medical record, monitoring the tracking board for potential admissions and complex patients, and updating the home medication record to match ordered and/or active prescription medications upon notification of patient admission. The MHS must possess excellent service skills, setting patients and families at ease with patience and compassion. They will work under the direction of Pharmacy Leadership, ED Leadership, Medication Safety Officer, and nurses to resolve discrepancies or errors in home medication orders.
