Medicare Annual Wellness Visit LPN
About The Position
Family Health Care is seeking qualified candidates for our Medicare Annual Wellness Visit (AWV) LPN position. This role supports BFHC’s Medicare population by coordinating and completing Medicare Annual Wellness Visits at assigned clinic sites. The LPN will conduct scheduled face-to-face individual Medicare Annual Wellness Visits in the clinic setting and, when approved by the Chief Medical Officer, via telephone. All required documentation, assessments, and clinical workflows must be completed in accordance with established BFHC policies, procedures, protocols, standing orders, and CMS guidelines. The LPN will perform all responsibilities according to best practices and reflect the mission and vision of Baldwin Family Health Care.
Requirements
- Current Michigan LPN license with at least two years of experience with adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting, within the past five years.
- Graduation from an accredited nursing program.
