About The Position

Patient Engagement Advisors (PEA) is seeking a full-time Transition Specialist to work onsite at AdventHealth Tampa. This position will also perform duties as an alternate Logistics Transition Specialist. The role involves setting patients up for success to transition out of the hospital into their next stage of care. The Transition Specialist will ensure the patient is provided services, educated on their discharge requirements (medications, patient follow-up care, nutrition, transportation, assistance programs, etc.), and equipped with the resources necessary to be successful in his/her next stage of care. The Logistics Transition Specialist is responsible for delivering medications, nutrition, product kits and other patient care related items as needed to patient’s bedside.

Requirements

  • High School Education or equivalent required
  • Must be 18 years old or older
  • 6 months pharmacy technician (retail preferred) OR 1-year medical assistant / patient care technician / patient care coordinator / clinic front desk associate / nursing assistant / home health aide / other healthcare or medical experience required
  • Retail pharmacy experience strongly preferred
  • Other related healthcare, hospital, or medical experience will be considered
  • Associates and/or Bachelor’s degree will be considered in absence of healthcare experience
  • Ability to report to work onsite as scheduled; ability to be on feet – standing, walking, and/or climbing stairs 8-10 hours daily
  • Valid driver’s license in the state of employment with a clean driving record
  • Ability to receive required vaccinations (as required by PEA or hospital) unless approved for exemption
  • Intermediate computer skills required
  • Web-based tools (including, but not limited to, point of sale, pharmacy information systems, and clinical/EMR systems)
  • Ability to utilize / navigate applications on an iPad

  • Ability to work a flexible and/or rotating schedule, including, but not limited to, nights, weekends, and holidays. Team members may be required to work during inclement weather (as directed by PEA and facility guidelines)

Responsibilities

  • Schedule patient’s follow-up appointments
  • Schedule patient’s transportation as needed
  • Deliver discharge prescription medications to patient’s bedside
  • Assist patients with medication affordability
  • Coordinate home meal delivery service for patients
  • Connect patients to community resources as needed
  • Delivery of medication, nutrition product kits and other patient care related items as needed to the patient’s bedside
  • Collects and documents social determinants of health information for every patient to whom services are offered
  • Provides pick-up and delivery of prescriptions and medical supplies in an accurate and timely manner
  • Ensures proper handling and storage of all medications and supplies
  • Obtains signature of person receiving the delivery
  • Collects payments; complies with payment collection related policies and procedures
  • Utilizes PEA software and assistance programs (coupons, vouchers, discount cards) to provide optimal savings/value for patient
  • Utilizes Countdown to Discharge (C2D) and other applications appropriately to document completed activities and fulfill patient needs
  • Meets KPI “Scheduled” goals and ensures follow up appointments are documented in C2D
  • Actively participates in hospital-specific rounds/huddles to obtain critical information and contribute as a collaborative member of the clinical care team/hospitalists
  • Submits one “Wow Story” based on PEA Leadership’s requested intervals, to Supervisor describing exceptional patient care experience provided
  • Works with prescribing clinicians and pharmacists to ensure affordability and successful delivery of medications per Standard Operating Procedures
