About The Position

Gather Health is seeking a Medical Technician Partner (EMT or Paramedic) to provide in-home care and support for senior patients. This role is crucial in bridging the gap between patients' social challenges (Social Determinants of Health) and their access to quality healthcare. The Medical Technician Partner will act as the eyes, ears, and hands of the medical teams in the patient's home, collaborating with primary care providers, Gather Guide Partners, and clinic administrators within a care-team model. The goal is to provide exceptional care and support with compassion, contributing to Gather Health's vision of creating equity in healthcare.

Requirements

  • Minimum of 1 year of experience working as an EMT or direct patient care.
  • Active certification as an EMT-B, EMT-A, or Paramedic in Massachusetts.
  • Active certification in AHA or Red Cross Healthcare Provider CPR.
  • Active unrestricted US driver's license.
  • Ability to work for any employer in the United States.

Nice To Haves

  • Experience working with senior populations, particularly with complex social needs.
  • Experience with basic office tools such as email, Outlook calendar, etc.
  • Strong verbal, written, and reading comprehension skills in English.
  • Secondary language capabilities.
  • Active certification as a Phlebotomy Technician.

Responsibilities

  • Provide in-home care and support for senior patients.
  • Serve as a clinical liaison between patients and primary care providers.
  • Perform approximately six to eight home visits per day.
  • Address acute medical decompensation or provide routine clinical care, including phlebotomy and point-of-care testing (EKGs, UAs, INRs, respiratory swabs, etc.).
  • Check in and organize an inpatient or ED admission avoidance plan.
  • Conduct social visits with patients in their homes.
  • Make outbound calls to patients to check in on their health status.
  • Utilize Electronic Health Record (Athena) to review patient appointments, charts, and document notes.
  • Leverage and access medical control through the Clinical Medical Director as needed to deliver care.
  • Additional duties as assigned.
