About The Position

Gather Health is a collective of outreach partners, care partners, clinicians, and patient partners working together to reshape lives and communities by bringing hands-on social and medical care to patients, both within care facilities and in their homes. This role, internally called “Medical Technician Partner” (MTP), will provide in-home care and support for older adult patients facing social challenges (Social Determinants of Health) such as lack of transportation. The MTP will act as the eyes, ears, and hands of the medical teams in the home, collaborating with primary care providers, Gather Guide Partners, and clinic administrators as part of a care-team model. The MTP will play a significant role in providing the best care and support possible with a deep sense of commitment and compassion, contributing to Gather Health's vision of creating equity in healthcare.

Requirements

  • Minimum of 1 year of experience working as an EMT or direct patient care.
  • Active certification as an EMT-B, EMT-A, or Paramedic in Massachusetts.
  • Active certification in AHA or Red Cross Healthcare Provider CPR.
  • Active unrestricted US driver's license.
  • Ability to work for any employer in the United States.

Nice To Haves

  • Experience working with the senior populations, particularly with complex social needs.
  • Experience with basic office tools such as email, Outlook calendar, etc.
  • Strong verbal, written, and reading comprehension skills in English.
  • Secondary language capabilities.
  • Active certification as a Phlebotomy Technician.

Responsibilities

  • Provide in-home care and support for senior patients and serve as their clinical liaison with primary care providers.
  • Perform approximately six to eight home visits per day, addressing acute medical decompensation or providing routine clinical care including phlebotomy and point of care testing (EKGs, UAs, INRs, respiratory swabs, etc.).
  • Check in and organize an inpatient or ED admission avoidance plan.
  • Conduct social visits with patients in their homes.
  • Make outbound calls to patients to check in on health status.
  • Leverage and access medical control through the Clinical Medical Director as needed to deliver care.
  • Utilize Electronic Health Record (Athena) to review patient appointments, charts, and document notes.
  • Additional duties as assigned.
