About The Position

Gather Health is seeking a Medical Technician Partner (EMT or Paramedic) to provide in-home care and support for their older adult patients. This role involves being the eyes, ears, and hands of the medical teams in the patient's home, working collaboratively with primary care providers, Gather Guide Partners, and clinic administrators. The Medical Technician Partner will play a significant role in providing the best care and support possible with a deep sense of commitment and compassion, ultimately contributing to Gather Health's vision of creating equity in healthcare. The role addresses Social Determinants of Health (SDOH) challenges, such as lack of transportation, by bringing clinical care directly to patients' homes.

Requirements

• Minimum of 1 year of experience working as an EMT or direct patient care.
• Active certification as an EMT-B, EMT-A, or Paramedic in Massachusetts.
• Active certification in AHA or Red Cross Healthcare Provider CPR.
• Active unrestricted US driver's license.
• Ability to work for any employer in the United States.

Nice To Haves

• Experience working with the senior populations, particularly with complex social needs.
• Experience with basic office tools such as email, Outlook calendar, etc.
• Strong verbal, written, and reading comprehension skills in English.
• Secondary language capabilities.
• Active certification as a Phlebotomy Technician.

Responsibilities

• Providing in-home care and support for senior patients and serving as their clinical liaison with primary care providers.
• Performing approximately six to eight home visits per day, addressing acute medical decompensation or providing routine clinical care including phlebotomy and point of care testing (EKGs, UAs, INRs, respiratory swabs, etc.).
• Checking in and organizing an inpatient or ED admission avoidance plan.
• Conducting social visits with patients in their homes.
• Making outbound calls to patients to check in on health status.
• Leveraging and accessing medical control through the Clinical Medical Director as needed to deliver care.
• Utilizing Electronic Health Record (Athena) to review patient appointments, charts, and document notes.
• Additional duties as assigned.
