About The Position

PCHC's Community Care & Geriatrics (CC&G) team is seeking a Medical Support Assistant (MSA) to serve as the communication and administrative backbone of a team dedicated to caring for older adults and patients with complex healthcare needs. In this role, you'll coordinate communication between providers, care teams, facilities, and patients to ensure information flows accurately and efficiently. You'll support providers who deliver care in community settings, manage critical documentation, and help ensure every patient interaction is handled with compassion, professionalism, and attention to detail. If you're organized, adaptable, and energized by meaningful work behind the scenes, you'll play an essential role in helping our patients receive the coordinated care they deserve.

Requirements

  • High school diploma or equivalent required.
  • Minimum of 1 years required, with 3 years of experience preferred, in medical office setting.
  • Patient Service Representative or equivalent experience preferred.
  • Preferred completion of Medical Terminology course within one year of hire.
  • Strong customer service skills required.

Responsibilities

  • Serve as the Communication Hub: Act as the primary liaison between Community Care & Geriatrics, the Patient Communications Center, healthcare facilities, and internal PCHC departments to ensure patient needs and provider communications are addressed promptly.
  • Support Providers Delivering Community-Based Care: Prepare provider schedules and documentation, pre-prep patient information, and ensure providers have the information they need to deliver exceptional care.
  • Manage Patient Documentation & Medical Records: Review provider documentation for completeness and accuracy, verify patient demographics and visit information, coordinate with providers to resolve missing information, and facilitate timely routing of records to Billing and Health Information Management.
  • Coordinate Office Operations: Answer office calls, monitor incoming faxes and electronic communications, route messages appropriately, and provide dependable administrative support to providers and clinical team members.
  • Ensure Accurate Patient Information: Verify patient records, maintain accurate documentation within the electronic medical record, and support processes that promote efficient, high-quality patient care.
  • Contribute to Continuous Improvement: Collaborate with leaders and cross-functional teams to strengthen communication workflows, improve documentation processes, and support operational excellence across Community Care & Geriatrics.
