About The Position

As an Inpatient Medical Social Worker, you will deliver comprehensive social work support by conducting psychosocial assessments, identifying patient needs, implementing appropriate interventions, follow-up care, and discharge planning. You will collaborate with interdisciplinary teams to coordinate safe, timely care across the continuum, including post-acute services and community resources, while advocating for patient-centered care. You will educate patients and families on healthcare decisions, available resources, and complex issues such as advanced directives, end-of-life care, and social concerns. You will connect individuals to community services and agencies, ensuring continuity of care and smooth transitions. You will support care coordination by managing length of stay, reducing readmissions, and ensuring appropriate levels of care within network systems. You will maintain clear communication, thorough documentation, and compliance with regulatory and professional standards, while also contributing to staff education and professional development. You will demonstrate knowledge of social work principles and methods and the ability to apply these in a health care setting. Your feedback matters. Requirements

  • IL locations-LSW Eligible, LCSW preferred issued by the State of Illinois
  • WI locations -Social Work Certification issued by the State of Wisconsin
  • Master’s degree in social work from a Council on Social Work Education (CSWE) accredited school.
  • 1 year of social work experience in healthcare
