About The Position

The Medical Social Worker is responsible for the continuous improvement of the total quality care as it relates to meeting the psychosocial needs of the patient/client. Services are provided in accordance with the patient's plan of care, accepted standards of professional practice, applicable law and regulation, as well as the organization's healthcare policies and procedures. Broward Children’s Center (BCC) is dedicated to transforming lives by providing specialized care to children and young adults who are medically fragile and developmentally disabled. Our comprehensive services range from 24/7 skilled nursing at our pediatric care facility to developmental education programs, therapeutic support, and family-centered services like respite care, social services, and in-home health support. Requirements

  • Master’s Degree in Social Work
  • a minimum of 1-2 years’ experience working with families and children in a healthcare environment.
  • Valid Florida Driver's License
  • CPR and First Aid Certification
  • HIV/AIDS Certification

Nice To Haves

  • Experience with the Adult Persons with Disability (APD) preferred.
  • Experience with Medicaid and Social Security processes and requirements preferred.

Responsibilities

  • Assess each patient’s unique condition, combining data from medical diagnoses, personal interviews and professional knowledge to establish accurate picture of overall mental and emotional health.
  • Make routine visits to assess resident’s psychosocial status.
  • Communicate with physician to report resident progress, needs, concerns, and potential and actual issues or concerns.
  • Maintain accurate and timely records including clinical and progress notes according to department policy.
  • Work closely with members of the health care team in the development of the plan of treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.
  • Develop working relationship with leaders in community resource groups, representing the organization at meetings, resource fairs and on various committees that address the social needs of residents such as Developmental Services, etc.
  • Participate in planning and implementing quality improvement activities.
  • Provide positive customer relations in dealing with residents, families, physicians, fellow department employees, other Skilled Nursing Facility departments, referral resources, service agencies, and others.
  • Assist with obtaining school-based program components.
  • Adhere to department and SNF policies.
  • Conduct monthly Resident Council Meetings in Group Homes and Skilled Nursing Facility
  • Develop and maintain a clear understanding of the parameters, guidelines and requirements of Home school or Community based school systems.
  • May be required to create and maintain Care Plans for each of the clients; assist parents in creating and maintaining Advance Directive and Guardianship plans
  • Must attend in-services and other professional development trainings
  • Other duties may be assigned.
