VNA is looking for a Medical Social Worker. The Medical Social Worker evaluates the social and emotional factors affecting a patient’s illness, care needs, ability to function, and capacity to cope with challenges in daily living with our Home Health and Palliative Care patients. Collaborates with the healthcare team to develop individualized treatment plans and actively participates in patient care conferences.
Requirements
- Bachelor of Science in Social Work with at least one year of healthcare experience, or a Master’s degree in Social Work
- At least one year of social work experience in a healthcare setting, preferably in home health
- Possess a current CPR certification
- Current California driver's license
- Auto insurance
- Reliable vehicle
Responsibilities
- Evaluates the social and emotional factors affecting a patient’s illness, care needs, ability to function, and capacity to cope with challenges in daily living with our Home Health and Palliative Care patients.
- Collaborates with the healthcare team to develop individualized treatment plans.
- Actively participates in patient care conferences.
