About The Position

The Case Manager LMSW is a professional who collaborates with physicians, health care team members and patient/families to coordinate care for specific patient populations across the continuum and between care settings during an episode of illness. This is a casual, part-time position with a rotating shift, working in the Case Management Department at Grand Haven Hospital.

Requirements

• Masters degree in social work required
• State of Michigan, social work license required

• Prior employment experience in a related area preferred

Responsibilities

• Identify, coordinate, communicate and participate in discharge planning with patients and a multidisciplinary team.
• Recognize and monitor potential barriers to safe discharge planning and intervene as appropriate.
• Navigate community resources as well as serve as a reference and liaison for community resources and services.
• Perform conflict resolution and crisis intervention when and where appropriate.
• Accurately document interventions, assessments, and discharge planning in the electronic medical record.
