Pinnacle Home Care, Florida’s largest independent Medicare-certified home health provider, is seeking a PRN Licensed Medical Social Worker (MSW) to join their award-winning team. This role focuses on delivering compassionate, patient-centered care in patients' homes across Flagler and Volusia Counties. Requirements

  • Master’s degree from a school of social work approved by the DCS, the Administrator, the Executive Director, and the Quality Assurance department staff.
  • Must have or be in the process of acquiring certification from the Academy of Certified Social Workers.
  • Two years’ experience with at least one year of experience in a health care setting (hospital, clinic, rehabilitation center, etc.).
  • The ability to communicate well, both verbally and in writing.
  • The ability to access clients’ homes, which may not be routinely wheelchair accessible.
  • Hearing, eyesight, and physical dexterity must be sufficient to perform a physical assessment of the client’s condition and to perform client care.
  • Physical activities will include walking, sitting, stooping, standing and minimal to maximum lifting of clients and the turning of clients.

Responsibilities

  • Assists in the admission process by performing an initial evaluation, assessing the client’s psychosocial status, and evaluating the client, family, and home to identify socioeconomic and emotional factors affecting the plan of treatment.
  • Assists in the development and implementation of the interdisciplinary client care plan as it pertains to medical social work.
  • Observes, records, and reports changes in the client's emotional and social factors that affect the client's illness, need for care, and response to treatment.
  • Consults with the attending physician concerning alteration of the treatment plan.
  • Maintains and submits written clinical records, including the initial evaluation, care plan, and daily notes.
  • Evaluates the clients' and family's response to, and effectiveness of, the medical social work intervention.
  • Confirms, on a weekly basis, the scheduling of visits with the Director of Clinical Services (DCS) to facilitate coordination of other staff visits.
  • Participates in Agency activities and committees, including staff development and in-service education.
  • Participates in discharge planning.
  • Assumes responsibility for self-development through formal education, workshops, conferences, professional meetings, and reading.
  • Assesses the client's potential to cope with social and health problems.
  • Acts as a consultant to health team members, assisting them in understanding social, emotional, and environmental factors related to health problems.
  • Helps clients utilize family and community resources.
  • Provides rehabilitative and supportive casework.
  • Assists clients and their families in coping with personal and environmental difficulties that may predispose them to illness or interfere with obtaining maximum benefits from medical care.
  • Maintains awareness of the Agency’s QAPI program and participates as required.
  • Manages and follows up on customer complaints according to the Agency’s complaint policy.
  • Maintains awareness of patient rights and the Agency’s responsibility in honoring them, as described in the Agency’s service agreement and admission packet.
  • Maintains awareness of appropriate reasons for patient discharge and the Agency’s policy on discharge.
  • Maintains awareness of potential signs of abuse and neglect and the steps required for reporting.
  • Designs and identifies patient-centered, individualized goals with realistic, measurable timeframes.
  • Maintains awareness of infection prevention and control techniques and participates in the Agency’s infection control program.
  • Acknowledges and demonstrates competence in the appropriate bag technique policy when applicable.
  • Utilizes CRT, calculator, multi-line telephone, copy machine, hand washing materials, and PPE.
