About The Position
Preferred Care Home Health is seeking a compassionate, experienced PRN Medical Social Worker to support patients and families navigating health challenges in the comfort of their own homes. This is a flexible, per-diem opportunity ideal for an MSW who wants meaningful, patient-centered work without a full-time commitment. As part of our home health team, you'll help patients cope with the emotional, social, and practical realities of illness or disability — connecting them to resources, supporting care planning, and empowering them to live as independently and fully as possible.
Requirements
- Master's degree in Social Work from a CSWE-accredited program (required)
- Minimum of 1 year of social work experience in a health care setting
- Strong assessment, communication, documentation, and care coordination abilities
- Comfortable managing multiple priorities and working independently
- Self-motivated, resourceful, and genuinely people-focused
Nice To Haves
- Home health experience a plus
- Florida social work license preferred, not required
Responsibilities
- Assess the social and emotional factors affecting a patient's health and recovery
- Help patients understand, accept, and follow their physician's care recommendations
- Guide patients and families through personal and environmental challenges that impact healing
- Provide services aimed at restoring patients to their optimum level of well-being
- Contribute to the patient's Plan of Care as part of the interdisciplinary team
- Identify and connect patients with community agencies, family support, and other resources
- Support discharge planning and serve as a consultant to fellow health care staff
- Complete clinical and progress notes within required timeframes
- Participate in team conferences, in-services, and Performance Improvement initiatives
- Uphold strict confidentiality for all patient, employee, and Agency information
