Requirements

  • Degree in Social Work (Minimum of Bachelor's degree in Social Work or Psychology)
  • LSW license in good standing in the State of Colorado
  • Documented evidence of training in dealing with grief and loss.
  • Hospice Experience (Required)
  • Bachelor's in Psychology or Social Work

Nice To Haves

  • Social Work License (Preferred)

Responsibilities

  • Provide emotional support to patients and families.
  • Refer patients to appropriate resources.
  • Advocate for patient needs.
  • Provide crisis intervention.
  • Educate families and patients on hospice and available resources.
  • Assess patients and create a care plan.
  • Develop a plan of care in collaboration with the interdisciplinary team to meet the needs of the hospice patient and family.
  • Assist hospice team members in understanding the social and emotional factors related to the hospice patient’s terminal illness.
  • Provide emotional counseling to hospice patients/families and assist patients/families in coping, decision making, advanced directives, funeral planning, or facility placement.
  • Conduct assessments of hospice patients’ and families’ psychosocial, financial, environmental, and community needs.
  • Make appropriate referrals to community resources based on the assessment of the hospice patient’s needs.
  • Comply with applicable legal requirements, company standards, policies, and procedures.
  • Complete all documentation in an accurate and legible manner.
  • Comply with hospice policies and procedures, laws and regulations, and standards of practice.
  • Conduct bereavement assessments and develop plans of care in conjunction with the IDT and document all bereavement activities.
  • Design and distribute materials to families eligible for and/or receiving bereavement services.
  • Develop educational programs for families and staff on loss, grief, and related topics.
  • Provide consultation and education to the community to increase knowledge related to hospice and serve as an information source and education resource person to the community.
