About The Position
This role is for a Home Health Medical Social Worker Intern (MSW) in Assisted Living Facilities. The position involves providing psychosocial support and social services to patients, identifying factors impacting patient outcomes, and collaborating with healthcare teams. Requirements
- Working toward a Master’s or Doctoral degree from a school of social work accredited by the Council on Social Work Education
- Minimum one year of social work experience in a healthcare setting required
- Strong assessment, communication, and care planning skills
- Ability to work independently while collaborating with an interdisciplinary team
- Valid driver’s license, reliable transportation, and current auto insurance
Nice To Haves
- Home health experience preferred
Responsibilities
- Assist physicians and interdisciplinary team members in understanding social and emotional factors impacting health conditions
- Develop and coordinate care plans addressing psychosocial needs
- Connect patients and families with community resources and support services
- Provide counseling, education, and emotional support to patients and caregivers
- Assist with discharge planning and care transitions
- Collaborate with physicians and interdisciplinary care teams to improve patient outcomes
- Maintain timely and accurate documentation in accordance with Medicare guidelines and agency policies
- Complete documentation efficiently using eClinicalWorks(eCW) EMR
