About The Position
At American Medical Hospice Care, we believe in providing exceptional, compassionate care to patients and families facing life-limiting illnesses. Our interdisciplinary team works together to support dignity, comfort, and peace in the final stages of life. We are seeking a dedicated and empathetic Medical Social Worker (MSW) to join our team and make a meaningful difference in the lives of those we serve.
Requirements
- Master’s degree in Social Work (MSW) from an accredited school.
- Licensed in the state of Texas.
- Valid driver’s license and reliable transportation; travel to patient homes required.
Nice To Haves
- Minimum of one year experience in medical, palliative, or hospice social work preferred.
- Strong interpersonal skills with a compassionate and non-judgmental approach.
- Knowledge of Medicare/Medicaid hospice regulations a plus.
Responsibilities
- Conduct psychosocial assessments of patients and families upon admission and throughout care.
- Develop individualized care plans addressing emotional, social, financial, and practical needs.
- Provide crisis intervention, grief counseling, and support for anticipatory loss.
- Assist patients and families with end-of-life planning, including advance directives and funeral arrangements.
- Advocate for patients and connect families with community resources and support services.
- Participate in interdisciplinary team meetings and coordinate care with nurses, chaplains, and physicians.
- Maintain accurate and timely documentation in compliance with regulatory and organizational standards.
