Medical Social Worker General Summary: Provide coordinated care in the home to clients of all age groups and perform psychosocial assessments, analysis, counseling and referrals to meet the needs of the client/family. Patient Population: Has contact with patients of all ages in a clinical setting; understands and demonstrates appropriate behavior when interacting with patients in all populations. Essential Functions: Assists in the admission process of the client to the Agency by performing an initial evaluation, assessing the client’s psychosocial, financial, environmental and community needs as evidenced by POC, documentation, clinical records, conferences, and community resource referrals. Demonstrate commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards and requirements. Collaborate with interdisciplinary team to promote coordination of patient care. Accept clinical assignments that are consistent with education and competence to meet the needs of the patients. Provides effective communication to clients, their families, team members, and other health care professionals to achieve desirable outcomes, and regular participation in Case Conference/Team Meetings. Demonstrates commitment and professional growth by participating in in-service programs and maintaining/improving competency. Meet mandatory continuing education requirements of the Agency/licensing board. Supervises: N/A Experience:

Requirements

  • (1) year experience as an MSW in a health care/hospice setting.
  • MSW skills as defined as generally accepted standards of practice.
  • Excellent interpersonal skills and ability to communicate effectively.
  • Demonstrates proven decision-making skills.
  • If required to make visits, Hepatitis profile.
  • Must read, write, and comprehend English.
  • Bachelors or Masters degree in Social Work.
  • Current driver's license in good standing. The employee is responsible for renewing their driver's license before it expires to continue employment.
  • Reliable transportation and auto liability insurance.
  • Current MSW Certification that is in good standing to practice as an MSW in any/all applicable states.
  • Current CPR certificate.
  • On-going employment as an MSW always requires the maintenance of a valid MSW Certification in the state where the employee practices and a valid CPR certificate. The employee is responsible for renewing these before they expire to continue employment.
  • Prolonged sitting, standing, and walking are required.
  • Ability to always handle stressful situations in a calm and courteous manner.
  • Requires working under some stressful conditions to meet deadlines and Agency needs.
  • Works in an office environment most of the time.
  • Available to Agency personnel in person or by telephone during the Agency operating hours and possibly after hours for emergencies.
  • Some exposure to unpleasant weather.
  • Ability to travel locally.
  • Reliable transportation and auto liability insurance.
