About The Position

Human Touch Home Healthcare is seeking a compassionate and dedicated PRN Medical Social Worker to join our interdisciplinary care team in Colorado. The Medical Social Worker is responsible for implementing standards of care for medical social work services in accordance with the state Medical Social Work Practice Act and supporting patients and families in navigating psychosocial, emotional, and environmental challenges related to illness.

Requirements

  • Bachelor's degree in Social Work from a program accredited by the Council on Social Work Education (CSWE)
  • Minimum of 1 year of experience in a healthcare setting
  • Strong verbal and written communication, documentation, and organizational skills
  • Current CPR certification (or ability to obtain upon hire)
  • Valid driver's license, reliable transportation, and proof of automobile insurance in accordance with company requirements
  • Occupational Therapist License (Required)

Nice To Haves

  • Master's or Doctoral degree preferred
  • Home health experience strongly preferred
  • Experience: Occupational Therapist and Home Health 1 year (Preferred)

Responsibilities

  • Assess the psychosocial status of patients related to illness and environment and communicate findings to the Registered Nurse
  • Conduct social evaluations and develop individualized intervention plans based on identified needs
  • Maintain accurate and timely clinical records, progress notes, and documentation for all assigned patients
  • Provide information, referral services, and resource coordination for patients, families, and caregivers
  • Serve as a liaison between patients/families and community agencies to support continuity of care
  • Maintain collaborative working relationships with clinical staff to support coordinated patient care
  • Develop and maintain relationships with public and private community agencies as referral resources
  • Participate in interdisciplinary care planning, case conferences, and discharge planning activities
  • Assist physicians and care team members in understanding psychosocial and emotional factors impacting patient health
  • Perform additional duties as assigned by the Clinical Supervisor or Clinical Director
