About The Position

Hospice Medical Social Worker - Provide individualized and coordinated hospice care to bring safety and comfort to the patients and families. You will have independence and flexibility in the field to visit patients one on one in their home environment. Our office is in Roseville, MN and the Social Worker will have a general territory within our service area where they will provide in-home hospice care to the patients. The territory for this position would primarily be in the Southern section of the Twin Cities Metro area. Interim HealthCare of the Twin Cities offers a wide variety of home care ranging from skilled nursing, physical therapy and other licensed health care services, which help with the every day tasks that let people continue living at home where they are most comfortable. Additionally, we offer hospice care in the home that can make loved ones as comfortable as possible and provide support to the family, allowing them to concentrate on spending quality time together. Requirements

  • Graduate of a Master’s program in social work accredited by the Council on Social Work Education.
  • Licensed to practice as a Social Worker in the state of Minnesota. License is active and in good standing.
  • Minimum of one (1) year's experience in health care.
  • Able to travel within geographic area serviced by office.
  • Valid drivers license, and proof of auto insurance are required.

Nice To Haves

  • Hospice experience preferred

Responsibilities

  • Conducts an initial psycho-social assessment of assigned patients/families within five (5) calendar days of admission to contribute to the comprehensive assessment of the patient’s/family’s needs and the Interdisciplinary Group (IDG) plan of care.
  • Provides individual and/or family counseling in response to the mental and emotional status of the patient/family to reduce barriers to achieving the goals of care.
  • Identifies special needs related to religious or cultural diversity and how these impact the implementation of the plan of care.
  • Evaluates identified financial or environmental concerns and directs the patient/family to appropriate community resources, as well as provides follow-up and acts as a liaison when indicated.
  • Provides crisis intervention support to the patient/family, members of a facility caregiver group or the IDG.
  • Assists the patient/family in electing advance directives, securing durable power of attorney, making funeral arrangements and other financial, legal and healthcare aspects of end-of-life choices.
