About The Position

The Medical Social Worker works under the direction of a physician and functions as an active member of the health team in accordance with physician orders and the written plan of care. The role provides patient services consistent with the hospice philosophy and assists the Interdisciplinary Team in understanding social, emotional, and cultural factors related to terminal illness. Responsibilities include conducting psychosocial assessments and evaluations and providing emotional counseling to patients and families.

Requirements

• Master’s degree in Social Work from a school accredited or approved by the Council on Social Work Education.
• Clinical experience relevant to the psychosocial needs of patients and families.
• Knowledge of psychosocial theories and use of psychosocial treatment techniques.
• Knowledge of cultural beliefs and practices related to death and dying.
• Excellent oral and written communication skills.
• Ability to work with dying patients and their families.
• Sympathetic attitude toward caring for hospice patients and positive communication skills when interacting with other members of the hospice team.
• Proficiency in Microsoft Windows 10 operating system.
• Valid driver’s license and auto insurance.

Nice To Haves

• ACSW or LCSW credential preferred, but not required.
