About The Position

Interim HealthCare is seeking a part-time Hospice Social Worker to provide home-based social services to patients and families facing end-of-life challenges in Hudson, Wisconsin, and surrounding areas. The role focuses on connecting patients and families with resources, counseling, and support, while advocating for their needs and helping them work toward care goals. The Hospice Social Worker works as part of a hospice team that may include an RN, LPN, CNA, and/or Chaplain. The position is listed as part-time, typically 2–3 days per week, with a day shift and an hourly salary range of $29.00–$35.00. Requirements

Master’s degree in social work.
Active MSW license in Indiana.
At least 1 year of social work experience.
CPR certification.
Knowledge of state and federal home health or hospice regulations.
Strong interpersonal and communication skills; empathetic, compassionate, and resourceful.

Responsibilities

Provide home-based social services to patients and families dealing with end-of-life issues.
Work with the hospice team to support the patient’s plan of care.
Monitor patients’ psychosocial conditions and identify social and emotional needs.
Conduct patient assessments, document progress, and ensure patients are moving toward their goals.
Provide counseling, community resource planning, crisis intervention, and advocacy.
Consult with family members and caregivers about the patient’s plan of care and ways to support progress.
