About The Position

Companion Hospice is hiring a Licensed Clinical Social Worker to support patients and families facing life-limiting illness. As a member of the interdisciplinary team, the Medical Social Worker helps patients and families address emotional, practical, and psychosocial challenges while supporting dignity, comfort, and quality of life. The role serves Guthrie, Edmond, Stillwater, and surrounding communities, with patient visits in homes, assisted living facilities, and nursing homes. It includes a company car and gas card, on-call duties as required, and work focused on hospice care rather than curative treatment.

Requirements

Master’s degree in Social Work (MSW) from a CSWE-accredited program.
Current Licensed Clinical Social Worker (LCSW) license in Oklahoma.
At least one year of experience as a social worker.
Excellent communication, interpersonal, and organizational skills.
Valid driver’s license and current auto insurance.
Ability to work independently in patients’ homes and community settings while collaborating with a team.
Commitment to the hospice philosophy of care.

Responsibilities

Conduct psychosocial assessments to identify patients’ and families’ emotional, social, financial, and community needs.
Provide short-term counseling, anticipatory grief support, and crisis intervention.
Collaborate with the Interdisciplinary Group (IDG) to develop and update the Plan of Care.
Connect patients and families with community resources and assist with Medicaid applications, advance directives, and POA/DPOA issues.
Participate in bereavement support as needed and complete timely, accurate documentation.
Contribute to discharge planning, continuous quality improvement, and staff education.
Perform on-call duties as required.
Travel to patient homes and community locations to provide services.
