About The Position

Croí Health is a mission-driven, non-profit organization providing home health, palliative, hospice, and wellness services across 27 communities. We are looking for an experienced, talented, and compassionate Hospice Social Worker for a part-time 24-Hour position at the Pat Roche Hospice House. The Hospice Social Worker will work as a part of a home-based interdisciplinary team providing counseling and targeted interventions for a population of homebound patients and their caregivers who are receiving Palliative or Hospice services through Croí Health. You will conduct an initial psychosocial assessment of the patient/caregiver contributing to the Agency's plan of care. The Hospice Social Worker strives for optimal patient outcomes with improved functioning and an increased ability to self-manage their bio-psych-social challenges. To provide medical services to Hospice patients/and or family members reflective of the philosophy, mission, and standards established by the Agency.

Requirements

Current MA license in good standing
Master of social work from an accredited program (MSW)
3 years + experience of social work experience
In-depth knowledge of social service skills and resources
Good communication skills, both verbal and written
Ability to harmoniously interact with patients/clients, families and colleagues
Handle pressure in various work environments and work in field
Ability to work independently with minimum supervision
Excellent organizational, flexibility and time management skills
Keeping up to date with education, policies and procedures

Nice To Haves

Hospice work a plus
Previous community health and medical health experience preferred

Responsibilities

Be flexible for on call holiday and weekend rotation.
