Medical Social Worker- PD
About The Position
The Medical Social Worker (LICSW) reports to the Manager of Case Management and Social Work. The role evaluates, diagnoses, and treats psychosocial problems using clinical social work skills to help patients and families address personal and environmental challenges associated with mental or physical illness and support their highest level of psychosocial functioning. Social Workers assigned to inpatient units participate in the co-management model with unit leadership to support patient flow. The role follows professional social work standards based on the NASW Code of Ethics. The position works in a normal office environment with exposure to patient care at Saint Anne’s Hospital in Fall River, Massachusetts. It is a per-diem role with daytime hours listed as 8:00 a.m. to 4:30 p.m., as required. The Social Worker provides functional guidance to Social Work Interns.
Requirements
- Master’s degree in Social Work from an accredited graduate school of Social Work.
- Current license from the Massachusetts Board of Social Work Licensure; LICSW required.
- Knowledge and skills to provide care with consideration of aging processes, human development stages, and cultural patterns throughout the care process.
- Between six months and one year of clinical experience, preferably in a health or mental health setting, with emphasis on populations served in the assigned area.
- Training and experience to address the needs of patients in assigned areas, including developmental stage, age-related issues, special needs, and cultural patterns.
- Analytical and therapy skills to evaluate patients, develop treatment plans, initiate therapy, and evaluate results.
- Ability to interact effectively with patients, families, medical staff, and outside agencies.
- Work is performed in a normal office environment with exposure to patient care.
Responsibilities

- Complete biopsychosocial evaluations to address barriers to treatment and discharge, improve patient health status, and support appropriate resource utilization.
- Consult with medical and support staff about patients’ backgrounds, psychosocial functioning, and needs to support comprehensive care planning, treatment compliance, care coordination, and patient satisfaction.
- Develop treatment plans; initiate individual and group clinical interventions; evaluate treatment progress and outcomes; and document social work activity in medical records according to departmental standards.
- Participate in collaborative care rounds and other conferences with care personnel and community agencies to coordinate care and support safe transitions to community and less restrictive settings.
- Educate patients and families to help them understand, accept, and follow medical and social work recommendations.
- Assess patient and family advocacy needs and coordinate advocacy within the hospital and community, emphasizing client self-determination and improved social supports and community resources.
- Provide field placement supervision and evaluation for BSW and/or MSW candidates in the department’s Clinical Social Work Student program, subject to the stated LICSW or experience qualification.
- Complete case-related statistics and other reports according to departmental standards.
- Participate in hospital-wide, departmental, and community studies, projects, and training related to psychosocial issues and barriers in service delivery systems.
- Stay current on community resources relevant to the clinical assignment and access them as needed.
- Maintain and enhance professional expertise through appropriate educational activities and attend training for specific clinical responsibilities as needed.
- Participate in quality assurance activities in accordance with department, hospital, and The Joint Commission standards.
- Follow Brown University Health, hospital, and department policies and procedures and applicable laws, regulations, and standards to maintain a safe care environment.
- Provide functional guidance to Social Work Interns, with recommendations to pursue LICSW-level licensure.
