Medical Social Worker - Social Services - Day Shift Including Every 4th Weekend Both Saturday and Sunday
About The Position

The Medical Social Worker plays a crucial role within the interdisciplinary team, focusing on ensuring the safe discharge of adult patients. This involves identifying patients with potential discharge barriers, conducting psychosocial assessments, documenting clinical information in Epic, and coordinating referrals to skilled nursing and rehabilitation facilities. The role also includes assessing cases of child abuse/neglect, domestic violence, elderly abuse, and sexual assault, as well as educating patients and families about post-acute care options, managed care, and payor issues.
Requirements
- Master’s Degree in Social Work (MSW)
- New York State License as a Licensed Masters Social Worker (LMSW).
- Communication and negotiation skills
- Ability to work independently and collaboratively with the interdisciplinary team.
- Work efficiently and effectively under pressure.
- Work comfortably within the limitations of the medical setting.
- Computer literacy.
Nice To Haves
- At least one year of experience in an acute care setting
- Discharge planning experience
- Experience with Epic and Allscripts
Responsibilities
- Identifies patients with barriers to a safe discharge plan.
- Completes psychosocial assessments on all patients.
- Documents all clinical assessments, notes, and referrals in Epic.
- Completes screens and coordinates all new skilled nursing facility and rehab facility referrals.
- Assesses all cases involving child abuse/neglect, domestic violence, elderly abuse, and sexual assault.
- Educates patient/family about post-acute care options and the role of choice.
- Educates patient/family about managed care and payor issues.
