About The Position
Guardian Angel Home Care is seeking an experienced Medical Social Worker (MSW) to join its home health team on a PRN / per diem basis. The role offers flexible scheduling, supplemental work, and compensation per visit or hourly depending on the assignment. Visits may be accepted based on availability, with additional assignments possible as patient census grows. The position is field-based and requires travel between patients’ homes throughout the assigned service area.
Requirements
- Master’s degree in Social Work (MSW).
- Full, active, unrestricted social work license; limited, temporary, associate, provisional, or restricted licenses are not accepted.
- Valid driver’s license.
- Reliable personal vehicle.
- Current auto insurance with the applicant’s name listed on the policy.
- Strong knowledge of community resources.
- Excellent communication and documentation skills.
- Ability to work independently in patients’ homes.
- Reliable, compassionate, and professional.

- Home health, hospice, hospital, rehabilitation, or community-based experience.
Responsibilities
- Complete psychosocial assessments.
- Evaluate patient and family needs.
- Provide counseling and emotional support.
- Connect patients with community resources and support programs.
- Assist with financial, transportation, housing, and long-term care resources.
- Support patients and families with adjustment to illness or disability.
- Participate in interdisciplinary care planning.
- Coordinate with nurses, therapists, physicians, and other clinical staff.
- Document visits and interventions accurately and timely.
- Travel to patients’ homes throughout the assigned service area.
