About The Position

Patriot At Home is seeking a per diem Medical Social Worker to join their Homecare team. This role involves providing social work services to patients in their homes within Stark and Medina Counties. The position offers a flexible schedule and is part of a veteran-owned and family-operated company that values its employees and community.

Requirements

Master’s Degree from a School of Social Work, approved by the Council of Social Work Education.
Must have, or be in the process of acquiring, certification from the Academy of Certified Social Workers.
Must have a criminal background check.
Must have current CPR (BLS) certification.
Ability to communicate well, both verbally and in writing, in English.
Ability to access patients’ homes, which may not be routinely wheelchair accessible.
Hearing, eyesight, and physical dexterity sufficient to perform a physical assessment of the patient's condition and to perform patient care.
Physical activities include walking, sitting, stooping, and standing and minimal to maximum lifting of patients and turning of patients.

Nice To Haves

Two years’ experience preferred.
At least one year of experience in a healthcare setting (hospital, clinic, rehabilitation center, etc.).

Responsibilities

Assist in the admission process by performing initial evaluations, assessing psychosocial status, and identifying socioeconomic and emotional factors affecting the plan of treatment.
Assist in the development and implementation of the interdisciplinary patient care plan as it pertains to medical social work.
Observe, record, and report changes in the patient's emotional and social factors that affect their illness, need for care, and response to treatment.
Consult with the attending physician concerning alterations to the plan of treatment.
Assess the patient's potential to cope with social and health problems.
Act as a consultant to health team members, assisting them in understanding social, emotional, and environmental factors related to patient health problems.
Help patients utilize family and community resources.
Provide rehabilitative and supportive casework.
Assist patients and their families in coping with personal and environmental difficulties that may predispose them to illness or interfere with obtaining maximum benefits from medical care.
