About The Position

As a per diem Medical Social Worker, you will provide psychosocial support, assessment, and care coordination to patients and families receiving services in the home. The role addresses emotional, social, financial, and environmental factors that impact health and recovery while supporting safe and effective care transitions. You will collaborate closely with the interdisciplinary team to connect patients with community resources and provide counseling.

Requirements

Master's and/or doctorate degree from a school of social work accredited by the Council on Social Work Education.
Active licensure in Medical Social Work (LMSW) or Clinical Social Work (LCSW).
Driver's license and access to reliable transportation.
Current CPR certification.
One year of social work experience in a healthcare setting.
Meet state and federal requirements for the position.

Responsibilities

Assist patients and families with health-related financial, social, and emotional concerns and help them develop effective coping skills.
Provide social work services in accordance with the plan of care.
Communicate changes in a patient's clinical status to other healthcare team members and care center office staff in a timely manner.
Educate patients, family members, and other team members on medical social work interventions and the treatment plan.
Perform care planning, transfer, and discharge functions.
Identify risks or threats to patient health in the patient's environment, promptly implement appropriate interventions to reduce risks, and communicate identified risks to the physician and other healthcare team members to coordinate care effectively.
Perform other duties as assigned.
