The Medical Social Worker will complete psychosocial assessments for patients' post-hospital care, assisting them and their support systems in coping with the stress of hospitalization, disability, and chronic/terminal illness. This role collaborates with the Clinical Care Management team to evaluate the appropriate level of care or environment setting for patient needs across the continuum. The social worker will serve as a resource to hospital staff and physicians regarding the emotional, social, and psychosocial aspects of a patient's illness and its impact on their social support system. They will provide information and education to patients on community resources and post-hospital care options, make referrals to community agencies, and report suspected cases of child and adult abuse, neglect, or exploitation. Additionally, the role involves serving as a liaison between the hospital, patients, and the Department of Social Services, and responding to consultative service requests after normal working hours.
