About The Position

Guardian Angel Home Care is seeking an experienced Medical Social Worker (MSW) to join its home health team on a PRN / per diem basis. This field-based role involves travel to patients’ homes throughout the assigned service area. It is suited to an experienced social worker seeking flexible supplemental work without a full-time commitment. Compensation is offered at a high per-visit rate, with hourly options potentially available depending on the assignment. Visits may be accepted based on availability, and additional assignments may become available as patient census and coverage needs grow.

Requirements

Master’s Degree in Social Work (MSW).
Full, active, unrestricted social work license; limited, temporary, associate-level, provisional, and restricted licenses are not accepted.
Valid driver’s license.
Reliable personal vehicle.
Current auto insurance with the applicant’s name listed on the policy.
Strong knowledge of community resources.
Excellent communication and documentation skills.
Ability to work independently in patients’ homes.
Reliable, compassionate, and professional.

Responsibilities

Complete psychosocial assessments.
Evaluate patient and family needs.
Provide counseling and emotional support.
Connect patients with community resources and support programs.
Assist patients with financial, transportation, housing, and long-term care resources.
Support patients and families as they adjust to illness or disability.
Participate in interdisciplinary care planning.
Coordinate with nurses, therapists, physicians, and other clinical staff.
Document visits and interventions accurately and promptly.
Travel to patients’ homes throughout the assigned service area.
