The Medical Social Worker (MSW) will be responsible for completing initial psychosocial evaluations by interviewing patients and their social support systems. They will provide counseling, support, and education to patients and their families to help them cope with the stresses of hospitalization, disability, and chronic or terminal illnesses. This role involves collaborating with patients, their social support systems, and a multidisciplinary team to develop and implement care plans that address identified psychosocial needs. The MSW will also serve as a resource for hospital staff and physicians, offering insights into the emotional, social, and psychosocial aspects of a patient's illness and its impact on their support system. They will intervene in crisis situations, complete psychosocial assessments for post-hospital care, and work with the Clinical Care Management team to determine appropriate care settings. Additionally, the MSW will maintain knowledge of community resources, provide information and education on these resources and post-hospital care options, and make necessary referrals. They are also responsible for reporting suspected cases of abuse, neglect, or exploitation and serving as a liaison with the Department of Social Services. This role identifies and addresses barriers to smooth transitions across the healthcare continuum and contributes to system improvements. The MSW will also develop and maintain accurate case records and document in the medical record according to departmental standards.
