About The Position

Guardian Angel Home Care is seeking an experienced Medical Social Worker (MSW) to join our San Diego home health team on a PRN / Per Diem basis. This position is ideal for an experienced social worker looking for flexibility, supplemental income, and strong compensation without a full-time commitment. We offer competitive high per-visit rates, hourly compensation options, flexible PRN scheduling, the ability to accept visits based on availability, and the opportunity for additional assignments as patient census grows.

Requirements

• Master’s Degree in Social Work (MSW)
• Active, FULL and unrestricted social work license
• Valid driver’s license
• Reliable personal vehicle
• Current auto insurance with your name listed on the policy

Nice To Haves

• Home health, hospice, hospital, rehabilitation, or community-based experience preferred
• Strong knowledge of community resources
• Excellent communication and documentation skills
• Ability to work independently in patients’ homes
• Reliable, compassionate, and professional

Responsibilities

• Complete psychosocial assessments
• Evaluate patient and family needs
• Provide counseling and emotional support
• Connect patients with community resources and support programs
• Assist with financial, transportation, housing, and long-term care resources
• Support patients and families with adjustment to illness or disability
• Participate in interdisciplinary care planning
• Coordinate with nurses, therapists, physicians, and other clinical staff
• Document visits and interventions accurately and timely
• Travel to patients’ homes throughout the assigned service area
