About The Position

The Medical Social Worker is responsible for providing social work services to patients in their homes per physician's plan of care.

Requirements

• Master's degree in Social Work (MSW)
• Current Ohio License in Social Work
• CPR Certification
• 1-3 years of job related experience
• One year experience in a health care setting (hospital, clinic rehabilitation center, home care, etc.)
• Must be capable of standing, stooping, bending and lifting up to 50 lbs.

Nice To Haves

• L.I.S.W OR I.S.W
• 3-5 years of job related experience
• 3-5 years of experience providing clinical services
• Experience in home care or community/outpatient setting preferred
• Computer skills preferred
