About The Position

In a career as a Medical Social Worker, you join us in providing life-changing care to patients from across the region. In this role, you will address financial and social concerns impacting care of Boys Town National Research Hospital and Clinic patients.

Requirements

  • Bachelor's degree in Social Work required.
  • Certification, Licensure, or Provisional Licensure as a Social Worker in the State of NE required.

Responsibilities

  • Facilitates care for the patient through referral to appropriate resources in the community or within the Boys Town system.
  • Provides leadership in the identification and acquisition of resources (local, state, and federal) for patients and their families.
  • Coordinates activities/follow-up to meet identified needs of select patients to facilitate necessary care, specialty consultation and/or surgery.
  • Serves as a resource for BTNRH physicians, nurses and staff.
  • Participates in multi-disciplinary teams as requested.
  • Maintains complete, accurate, and timely documents of completed work, patient files and office files.
  • Complies with all applicable statutes, rules, regulations, policies and procedures.
  • Maintains working knowledge of relevant medical/legal issues that impact patient care.
  • Participates in quality improvement activities through staff meetings, committees, continuing educational and professional activities.
  • Participates in Utilization Review Committee and activities.
  • Assumes the duties of the discharge planning coordinator.
  • Will provide oversight of the discharge planning process.
  • A daily review of the active patient discharge plans will be completed as needed.
  • Reassess the discharge planning process on an on-going basis (i.e., at least quarterly), review discharge plans in closed medical record review to determine staff responsiveness to the patient’s post-discharge needs and review re-admissions for trends.
