About The Position

LifeChoice Hospice & Palliative Care is seeking an experienced Hospice Social Worker to join its interdisciplinary team in Palatine, Illinois. The Social Worker supports hospice patients and families with emotional, social, financial, and practical needs throughout end-of-life care. The role works closely with nurses, physicians, chaplains, CNAs, families, facilities, and other hospice care team members. The organization is nurse-led and serves patients and families throughout the Chicagoland area, emphasizing compassionate, responsive care and teamwork.

Requirements

• Master's degree in Social Work.
• Current Illinois social work license appropriate for the position.
• Previous hospice social work experience.
• Strong knowledge of hospice and end-of-life care.
• Strong communication and documentation skills.
• Ability to work independently and as part of an interdisciplinary team.
• Reliable transportation for patient and facility visits.

Responsibilities

• Complete psychosocial assessments and ongoing social work evaluations.
• Provide emotional support, counseling, education, and resource coordination.
• Assist patients and families with end-of-life planning and changing care needs.
• Identify social, financial, environmental, and caregiver concerns.
• Connect patients and families with appropriate community resources.
• Participate in interdisciplinary team meetings and care planning.
• Maintain accurate and timely clinical documentation.
• Communicate closely with patients, families, facilities, and the hospice team.
• Assist with advance care planning within the social work scope of practice.
• Manage an assigned caseload and complete field visits as needed.
