About The Position

Northern Illinois Hospice is hiring a PRN Hospice Medical Social Worker to provide patient-centered support to patients and families during the hospice journey. The role focuses on offering emotional support, guiding families, advocating for patient needs, and helping ensure comfort during critical moments. The Social Worker collaborates with nurses and the interdisciplinary care team and supports advance care planning. The schedule is PRN.

Requirements

• Master’s Degree in Social Work (MSW).
• At least 1 year of experience as a Social Worker; healthcare or hospice experience is preferred.
• Strong communication, empathy, and organizational skills.
• Valid driver’s license and reliable transportation.

Nice To Haves

• Illinois LSW or LCSW.
• Healthcare or hospice experience.

Responsibilities

• Complete psychosocial assessments in hospice care.
• Develop care plans and provide counseling.
• Coordinate community resources and referrals.
• Support advance care planning, including DPOA-HC.
• Maintain timely and accurate documentation.
• Collaborate with the interdisciplinary hospice team.
• Ensure compliance with Medicare and Illinois standards.
• Meet with patients and families, provide emotional support, and guide them through the hospice journey.
• Advocate for patient needs and help ensure comfort during critical moments.
