This role helps patients honor their goals and wishes, promotes dignity at end of life, and offers emotional, social, and resource-based support throughout the hospice journey. The Medical Social Worker collaborates closely with the interdisciplinary team to enhance comfort, quality of life, and overall patient and family well-being. Responsibilities include counseling, resource navigation, advocacy, and coordination with community agencies, along with maintaining timely documentation and meeting organizational productivity expectations. The ideal candidate demonstrates strong communication skills, empathy, professionalism, and the ability to adapt to the unique needs of patients, families, and the care environment.
